Hmong Youth and Parents United (HYPU) is seeking a dedicated and skilled Communications Coordinator to join our team. As a community-based organization committed to empowering underserved communities, we are looking for a passionate individual to help us amplify our message and engage with our diverse stakeholders.

WORK DESCRIPTION

Under Supervision, the Program Coordinator plays a crucial role in ensuring the smooth and efficient operation of client intake and resource referral services. This position is responsible for coordinating all aspects of the client intake process, including initial assessments, eligibility determinations, and connecting clients with appropriate community resources.  The Program Coordinator will work closely with clients, staff, and external organizations to streamline intake procedures, manage case referrals, and ensure resources are effectively allocated to meet the needs of the population served.

GENERAL ROLES AND RESPONSIBILITIES

  • Client Intake:

    • Conduct initial client interviews to assess needs and eligibility for programs.

    • Gather and document comprehensive client information, including medical history, functional limitations, and social and economic circumstances.

    • Maintain detailed and up-to-date client records in accordance with agency policies and confidentiality standards.

  • Resource Navigation:

    • Research and identify appropriate community resources and services for clients.

    • Develop and maintain strong relationships with community partners.

    • Provide clients with information and referrals on a variety of topics, including housing, healthcare, employment, food assistance, and financial assistance.

    • Assist clients in accessing and utilizing available resources.

  • Program Coordination:

    • Oversee the day-to-day operations of the intake and resource referral processes.

    • Develop and implement intake procedures and protocols.

    • Track client progress and program outcomes.

    • Coordinate with other staff members and departments within the organization.

    • Participate in staff meetings and trainings.

  • Outreach and Community Engagement:

    • Assist in the development and execution of outreach strategies to promote awareness of available services and resources

    • Assist in the coordination of resource fairs, tabling, and outreach to support program goals. 

    • Build relationships with community partners to expand resource networks and improve access for clients.

    • Engage in community events or activities that may support the program's objectives.

  • Data Management and Reporting:

    • Collect and analyze data related to client demographics, service utilization, and program effectiveness.

    • Prepare reports for funders, supervisors, and other stakeholders.

    • Maintain accurate and timely records of all client interactions and services provided.

  • Other Duties as Assigned:

    • Participate in agency-wide committees and initiatives.

    • Assist with special projects as needed.

    • Any other duties that the direct manager assigns

QUALIFICATIONS, SKILLS, AND ABILITIES

  • Bachelor’s degree in Social Work, Human Services, Public Health, Nonprofit Management, or a related field (required).

  • Minimum of 2-3 years of experience in program coordination, case management, or related roles (preferred).

  • Strong knowledge of disability rights and relevant legislation (e.g., ADA, IDEA, IDD...)

  • Knowledge of relevant laws, regulations, and ethical standards related to client services and intake processes.

  • Ability to work independently and as part of a team.

  • Strong ethical and professional conduct.

  • Bilingual/Multilingual skills, preferably in Hmong. 

  • Excellent interpersonal, communication, organization, and time-management skills. 

  • Expertise in addressing community needs, including strategies to mitigate stigma and improve access to education for marginalized and underserved populations.

  • Proficiency in Microsoft Office Suite, Google Workspace and CRM software

PHYSICAL DEMANDS

  • Ability to sit or stand for extended periods.

  • Ability to use a computer for extended periods.

  • Ability to communicate effectively both verbally and in writing.

  • May require occasional travel within the community.

REQUIREMENTS

  • Must have a valid California Driver’s License with current auto insurance and reliable transportation.

  • Must be able to work nights and weekends as needed.
